Medical Coverage

Wood County's medical coverage is self-insured and utilizes a Provider Network to access discounted fees for service.  Subscribers will be protected from balance billing if using a participating (in-network) provider.  Refer to the Health Benefit Guide or Plan Document for the schedule of benefits.

Visit the FrontPath website to view participating providers.  The website also includes a Cost and Quality Portal where members can compare cost and quality for specific procedures based on providers in the network.  Contact the Benefits Line at 419.354.1373 for the user name and password for the site.

Precertification Number: 1.800.242.1199   Refer to the Plan Document for services that require precertification.

Participating providers will file your claims on your behalf. Should you have to file a claim yourself, you must submit a Paper Claim Form and itemized copy of billed charges to the address on the form/id card.
